HSCMH joins the Wyoming Rural Care Transition Program


(HSCMH August 30,2013)The Wyoming Rural Care Transition Program is a statewide patient-centered program designed to improve quality, reduce re-hospitalization and contain costs for patients who are 65 years of age and older with at least one of 10 qualifying diagnoses. This program provides Care Transition Nurse Coaches to help guide patients as they transition across multiple health care settings.
During an episode of illness, you may receive care in multiple settings, often resulting in incomplete or confusing information during or after discharge. Because you and/or your family/caregivers are most involved with your care, this program focuses on providing you and your family/caregiver with information necessary to manage your own care, improve your quality of life , and know where to go for assistance within the health care system.
During the 30-90 days following discharge, a Care Transition Nurse Coach will provide you with specialized written material, information and self-management skills to ensure you needs will be met when you receive additional care in multiple settings in the future. This program is provided at no cost to participants.
